The absolute biggest scare I've ever experienced in a Silent Hill game was a bug in Origins where the protagonist's face would show up deformed from the inside of his head because of how much the camera would zoom in upon entering certain rooms.

That shit made my heart leap the first time while playing at 2am on the PSP.
