In the original trilogy I always imagined he looked like this because after using the force for many dozens of years his body had started to decay and wear away from the corruption of the Dark Side. It was so dumb that the actual answer was because he accidentally hit himself in the face with lightning bolts. The fucking prequels, man.
I think it far more likely your first guess is correct, and his lightning being deflected was his way of purposefully removing his guise in a manner that made him out to be the victim to Anakin.
