Experienced ass eater here who's familiar with it. Ass eating can lead to getting hepatitis if the person whose ass you're feasting on is dirty, obviously. You can greatly minimize risk by having them clean out using an enema bulb and then double cleaning with soap and water.
You can even go a step further and use a condom cut sideways and placed over the anus and go to town that way which will definitely cut out all the risk. (But this doesn't feel as great for the person being rimmed, so your milage may vary on how long that's gonna be an option.)
