i don't work in the field directly anymore but physical review copies were usually dev carts for stuff that still used carts or just... burned cds with the game on them that only worked in devkit consoles. Much more common during PS3/360/3DS days...
eh it was both. you'd get debug builds for long-lead reviews, but lots of publishers sent plenty of final copies out.
so I'd have the disc to write the review off, but come a release day or just prior, a box would show up with a copy of the game on each platform.
