The fact that "Fiona" isn't actually a character from Bloodborne is the interesting part. That's what makes it less of a direct reference to the first game and feel more like a tease.

This is the big thing. Miyazaki always has a big hand in item descriptions and naming. He's not writing the descriptions for Sekiro but developed the overarching plot. So it stands to reason that he did the writing for Deracine. Why would he put/allow this(especially the unfinished tale portion) if he doesnt want to plant an idea in our minds. It doesn't have to be a doll of another doll, and in fact, that makes very little sense to begin with. Fiona is probably an actual person.
