It's a picture of a distressed woman that was murdered.
I imagine it's the pictures her killer took of her moments before actually killing her.
The end is still scary to this day regardless of your age. They set it up perfectly at the beginning and then don't really touch on it again until the end. You hear about that story of the guy who was inspired/possessed by the witch and he'd lure two kids into a house and then kill them. But he didn't want the kids to watch him kill each other so he'd have one turn their back while he killed the other. So the moment you see him with his back turned your mind immediately remembers that story.
There's also a real sense of simplicity to the whole scene. There's no music or just scare. Which is really rare for horror movies. Especially today when everything has to have creepy music and then a stinger when the scary moment is meant to occur.
