The first enemy is specifically not a flying lunatic, madman, or daredevil. The word "Maniac" is very specific here.

Well, the game I'm thinking of is older than Deus Ex. But newer than Doom.
welp, the last time I played that game was about 15 years ago 🤷
For a second I was thinking it could be one of the Dead or Alive games where Helena could be your first opponent and the last boss is a clone of Kasumi
Nope. Two hints the game is only ten years old and an RPG. I think people forget the first enemy is a crab as it's not the main enemy type.
