What this really speaks to is what the Trump team think about their base. That's who this is for, and if you are part of that base, yes, they do think you are this stupid.
To collect emails and contacts for donation/support begging from the people who respond to these things seemingly in earnest. If anything, joke replying with "Fuck Trump" in the 'Other' fields just saves them money on e-blast and marketing costs since they know they definitely don't need to target you.
That said I would not be shocked to learn that Trump gets a report on these so that "See sir? America really loves the work you are doing" even though the person who made this absolutely knows what they're doing.
These "surveys" are really just a way for them to add people to their email newsletters to encourage engagement and regular donations. The intent isn't to sway undecideds, it's to remind those who are already decided to vote.
