What is extreme violence supposed to be to get an AO rating? Many M rated games have had some already gruesome violence in them so I would love to know what crosses the line.

Manhunt 2 had to get violence censored in order to get an M. Same with that Punisher game.

I know there's a market for AO games but they aren't usually concerned with getting rated by the ESRB.

There literally can't be. Pretty much all stores cannot sell AO rated games, and I'm pretty sure all platforms ban them from their storefronts.

So the only way to get them is downloaded directly, and usually those don't get rated in the first place.
