Where did this meme start? Def not American (that commercial seems Aussie, maybe?). Given all the vids, this also seems like a recent, fresh meme. A GREEN meme.
These are my favourites. The ones that are just absoluely nonsensical and stupid in every possible way, the more you fight it the stronger it gets. Just fucking dumb, I love it.

Also, far more interesting than the meme, why the stretcher? How'd the man break his damn back falling into river? How high did he fall from? What's the story there?
